Prof Beverly Alimo-Metcalfe, PhD, MBA, MSc,
FRSA, FBPS, CPsychol.
Chief Executive, Real World Group
Beverly has an international reputation in the field of leadership studies - an area of interest for over 30 years. She is passionate about embedding ethical leadership throughout organisations, and in supporting individuals and organisations in strengthening their capacity for engagement, collaboration, innovation, leading change, and effectiveness.
As a result of her reputation she has undertaken numerous advisory roles for the UK government including the expert advisory panel of the National Health Service’s QIPP, Police, National College for School Leadership, as well as the Academic Advisory Panel of the Chartered Management Institute. She is currently a member of the Government’s “Guru Group” in relation to the Macleod Review on Employee Engagement, and is a Patron of Women in Management.
She has been working closely with the NHS and wider public and private sectors in research, postgraduate teaching, and leadership development since 1984 when she joined the Nuffield Institute for Health, University of Leeds. In 2001, while holding the Chair of Leadership Studies at the Nuffield Institute she conducted a 3-year ground-breaking research study into the nature of leadership. This led to the creation of a University of Leeds spin-out company, Real World Group. The company enables her to continue research into leadership, culture, and diversity, create robust diagnostic instruments; and undertake major leadership and cultural transformation projects across organisations.
Recently, she and colleagues together with King’s College London, completed a longitudinal research study funded by the Department of Health, which identified the link between particular leadership culture and the effectiveness of multi-professional teams in health. This is one of the first studies ever undertaken to show a direct causal link between leadership and productivity.
Beverly is now Professor of Leadership Studies at the University of Bradford, School of Management. She spends much of her time writing articles and presenting at conferences nationally and internationally.
Prof Bernard Crump

Bernard Crump studied Medicine at Birmingham University qualifying in 1980. After 7 years in clinical practice he returned to postgraduate training in Public Health Medicine. He spent a decade as DPH. In 2002 he became CEO of Shropshire & Staffordshire Strategic Health Authority, a post which he occupied until being appointed the first CEO of the NHS Institute for Innovation and Improvement in July 2005.
He has written and lectured on a wide range of topics in healthcare including aspects of population health, the role of clinicians in management, health and healthcare improvement, the use of metrics in encouraging improvement and the use of health economics in decision making. He is visiting professor at the University of Leicester and Honorary professor at the University of Warwick.
Dr Paul Barach, B.Sc., MD, MPH, Maj. (ret.)

Dr Barach has over 15 years’ experience in researching, teaching and applying human factors and system analysis in understanding healthcare provider and team communications and learning. He is visiting professor at the University of Utrecht, Netherlands, and Associate Professor at the University of South Florida, US, and was Professor and Director of the New South Wales Injury Research Center.
He has been integrally involved as a clinician, educator, researcher and policy maker in enhancing healthcare improvement and patient safety policy in the US, Europe and Australia. He is a board-certified Anesthesiologist, with fellowship training in Cardiac Anaesthesia and Intensive Care Medicine, and patient safety fellowship at the Massachusetts General Hospital and Harvard Medical School. He spent 6 months studying quality improvement with Dr Brent James at Intermountain Health Care, Utah. Dr Barach was editor of the lead journal Quality and Safety in Healthcare from 2001-2005.
He leads the Trauma Collaborative Injury Research Program at the NSW Health Department and the Life Time Care Authority, (http://traumacollaborative.com/), and is the Project Leader for the NSW Health Department project on improving the quality of health facility procurement and design. He leads the European Union FP-7, HANDOVER research and improvement project focused on developing better organizational and informational tools to improve patient discharge and transitions (www.handover.edu).
His major research focus is on developing and extending theoretical work in the areas of injury prevention, patient safety and quality improvement using both quantitative and qualitative methods. He has been awarded over $10 million in federal funding from US, EU and Australian national funding bodies. He has spent the last several years as co director of the Center for Heath Design Research Council funding and assessing research around the role of evidence based design and the impact of the built environment on patient outcomes. He is a member of the US based Hospital Design Facilities Guidelines Institute.
For several years he has co-taught a course in the Advanced Training Program at Intermountain Health Care in Salt Lake City, Utah; developed and taught a Patient Safety Certificate Program for the Quality Colloquium at Harvard, and has taught numerous workshops for the Institute of Healthcare Improvement’s National Forum and the BMJ International Forum. He has authored over 200 peer-reviewed papers and book chapters on clinical governance, risk management, quality improvement and patient safety.

Dr Jason Cheah
CEO, Agency for Integrated Care, Singapore
Dr Jason Cheah graduated with a medical degree from the National University of Singapore in 1991. After completing the requisite clinical training and residency postings in general internal medicine, he was offered a scholarship and traineeship in Public Health Medicine. He successfully completed that training in 1998 and obtained his Masters of Medicine in Public Health (with Distinction) from the same University. He concurrently also completed his Masters in Health Care Management from Wales University (Swansea) and a MBA from Warwick University. In 2001, he was awarded the Fellowship of the Academy of Medicine (FAM) of Singapore.
Dr Cheah worked for 8 years in the Ministry of Health Singapore, dealing with a variety of health policy and planning, health regulation and accreditation issues. He then spent a year in the private sector as the Assistant General Manager and then Acting General Manager of Gleneagles Hospital. In 2000, Dr Cheah returned to the public sector and started the Clinical Project Management and Planning Office in the National Healthcare Group (NHG). Dr Cheah was appointed as Chief Projects Officer (CPO) and also concurrently as Director of Clinical Services at Alexandra Hospital. In Nov 2005, Dr Cheah was appointed as Chief Executive Officer of the National Healthcare Group Polyclinics (NHGP).
In July 2009, Dr Cheah was appointed by the Ministry of Health Singapore as CEO of a new National Agency for Integrated Care (AIC). This Agency is charged with integrating care for the numerous patients with chronic medical conditions across the continuum from acute hospitals to community hospitals, nursing homes and home care.
Dr Cheah remains active in teaching in the Department of Epidemiology and Public Health in the Faculty of Medicine in the Yong Loo Lin (YLL) School of Medicine, National University of Singapore. Most recently, Dr Cheah has been co-opted to be an Associate Editor in the International Journal of Integrated Care (IJIC). He is also Chair of the Organising Committee of the 1st Asian Conference on Integrated Care held in Singapore in February 2011.

Steve is a highly experienced consultant with a practical and pragmatic approach to delivery & implementation. Prior to providing consultancy works, he was a senior engineer working for Toyota in the UK and Japan on business development to deliver greater efficiency, quality and profit returns. He was the Lead Engineer for the last generation corolla model, leading the vehicle project from drawing to volume production with a 33% cost reduction in production costs. Following Toyota and a stint in a large American Consultancy group, he set up KM&T in 2004. Now, he has accumulated 9 years of consultancy experience working in all major sectors (Manufacturing, MRO, Healthcare, Service & Public Sector) on high profile change projects and strategy. He works with client senior management level on: strategy, diagnostic, gap analysis, feasibility, due diligence, restructuring, planning, change management and development of programmes to maximise performance. Dr Sarah Muttitt

Dr. Sarah Muttitt brings more than twenty five years experience in the healthcare industry with a clinical background in pediatrics and neonatology and private/public sector experience in the development and implementation of innovative healthcare technologies.
Prior to moving to Singapore. Dr. Muttitt was the Vice-President, Innovation and Adoption at Canada Health Infoway, the organization providing strategic leadership for EHR implementation in Canada.
Dr. Muttitt joined Singapore's Ministry of Health Holdings in December 2007. As Chief Information Officer, she is responsible for national alignment of health IT initiatives with a focus on electronic health record solutions and their adoption by clinicians to support current and emerging health care challenges and priorities.

She is a certified facilitator of the Appreciative Inquiry for Societal Change program conducted by CWRU and has also been accredited facilitator for numerous other psychometric tools and 360-degree instruments. She has recently partnered with Real World Group from U.K. to bring Engaging Leadership to Singapore and has since introduced this ground-breaking body of research to numerous public and private sector organisations. She is also the founder of Imagine Singapore and contributes actively in international organizational development communities as a speaker, facilitator and resource person.

Ms Juliette Alban-Metcalfe,

Juliette is an Organisational Psychologist and Managing Director of Real World Group. She has a particular interest in leadership, diversity and inclusion. Over the past 12 years she has designed and published various research studies designed to better understand how organisations can best leverage potential in leadership, diversity and inclusion. She and colleagues have applied the findings to organisations with great success.
Juliette has developed organisational culture assessments, and co-authored 360-degree and team feedback instruments for leadership development. She has also been commissioned to produce publicly-available best practice guides for diversity and inclusion by voluntary sector and government bodies for the public and private sectors.
As a result of her research and experience in working with organisations, Juliette has been invited to speak at international conferences. She is the author of various peer-reviewed articles in international journals and book chapters on leadership. She has an MSc in Organizational Psychology from the University of London, Birkbeck College, and a Master of Science in Positive Organization Development and Change from Case Western Reserve University, Ohio.
In Singapore, Juliette and colleagues are partnering with Sequoia Group (SG) to bring the Real World Group engaging leadership model to organisations in a culturally-appropriate way for the ongoing success of the key services across the country.

Raymond Tung has over 15 years experience in manufacturing sector, supply chain and cleanroom environment. He has strong career progression from engineer to director level. He has also been enlightened by attending certified courses in Finance and Regional Security; as well as a course in Effective Leadership & Change Management, which was conducted personally by John Kotter, Emeritus, Harvard Business School, who is regarded as the world's foremost authority on leadership and change.
Sports has been an essential part of Ben's life, particularly in the areas of endurance sports, martial arts and chess.

Dr Jerome Goh

Dr Jerome Goh is the Head of Department in the Department of General and Forensic Psychiatry in IMH and the Clinical Director of Prison's Psychiatric Housing Unit. He obtained advanced specialist qualifications in Psychiatry in 2006 and graduated with a Masters degree in forensic psychiatry from the Institute of Psychiatry at King's College, London, in the following year. He is also involved in IMH's clinical quality improvement efforts and is currently the Chairperson of IMH's Clinical Risk Management Committee. He has an interest in the evidence-based practice of violence risk assessment and management and has certified training in instruments for assessing violence risk. He is also a certified facilitator for an offender treatment programme, the Reasoning and Rehabilitation Programme Revised (R&R).

Dr Ling Moi Lin
Director, Infection Control, Singapore General Hospital
Dr Ling Moi Lin received her medical education at the National University of Singapore. She obtained her postgraduate training in Microbiology at the Victoria University of Manchester, is a member of the Royal College of Pathologists of Australasia, and is a certified professional in healthcare quality (CPHQ). From 2002-2007, Dr Ling played a key role as Director of Quality Management at the Singapore General Hospital (SGH), contributed to the development of the quality and patient safety program as well as the implementation of clinical quality improvement program in SGH.
She is currently the Director of Infection Control at the SGH where she played a key role in the development of infection control program in the hospital.
She is the President of the Infection Control Association (Singapore) that was formed in 1999; and President of the Asia Pacific Society of Infection Control (APSIC). She was a Director on the Healthcare Quality Certification Board (HQCB) and its Asia Pacific representative from 2006-2007. She helped to set up the Healthcare Quality Society of Society in 2007 and is its founding President. Membership to other professional societies include the Hospital Infection Society (UK) and fellow of the Society of Healthcare Epidemiology of America (SHEA). She is currently the President of the Singapore Society of Pathologists. She is also a member of the Editorial Council of the International Journal of Infection Control.
Her interests are in infection control, antimicrobial resistance, molecular epidemiology as well as quality improvement. She is an experienced trainer in infection control, quality improvement, LEAN and root cause analysis (RCA). She has been conducting RCA courses for the Ministry of Health in Singapore since 2006 as well as for the 2nd International Conference of the Asia Pacific Society of Healthcare Quality (APSHQ) in 2008.

Dr Sandhya Mujumdar,
Deputy Director, Medical Affairs, National University Hospital
Dr Sandhya Mujumdar is a Deputy Director in the Medical Affairs department, National University Hospital (NUH), in Singapore. She is MD in Paediatrics from India and has a Master's degree from UK in Healthcare Management.
Dr Sandhya is responsible for clinical quality, clinical audits, clinical service, risk management, patient safety, health services research, outcomes and performance management, clinical pathways and case management in NUH. She, in collaboration with NUH team, has developed quality and safety framework in NUH. She has been actively involved in coordinating and implementing various quality and safety initiatives for NUH. She also has a rich experience in the similar and research fields from other hospitals in Singapore, where she has assisted in developing quality audit systems.
She is the JCI coordinator and facilitator for NUH. She was instrumental in preparing NUH to attain Joint Commission International (JCI) Accreditation in 2004 and recertification in 2007 and 2010. She has provided support to other hospitals in how to prepare for JCI in and outside Singapore and also helped to conduct tracer audits.
She has led and successfully completed many clinical quality improvement projects, including some rapid improvement events (RIE) that require applying the use of PDCA, lean methodology and lean tools.
Her key strengths are in teaching risk management, patient safety, root cause analysis, healthcare failure mode and effects analysis, quality improvement tools, lean methodology, clinical pathways, performance management, JCI standards, and tracer methodology.
She has given multiple presentations at conferences, presented posters on quality and safety, published articles and is the co-editor of the in-house NUH Quality and Patient Safety newsletter. She is one of the founding members of Healthcare Quality Society of Singapore (HQSS) and is actively involved in various activities conducted by the society.
She has given talks as an invited speaker at national and international conferences and at other international hospitals.

Ms Chook Mee Lan

Ms Chook Mee Lan received both her Bachelor of Science and Masters of Science degrees at the National University of Singapore. She is currently the Head of Facilities Services and Biosafety at Temasek Life Sciences Laboratory (TLL). She has been actively involved in biosafety from 2003 when she helped to oversee the construction of the TLL BSL-3 and ABLS-3 laboratories from design to handover. She is currently the Asia-Pacific Biosafety Association (A-PBA) Secretary, a post she has held since 2008. She has also been a trainer for the annual A-PBA Biosafety Management Courses, as well as the National Institute of Health Trainer the Trainer (TTT) programme and other biosafety workshops in Laos and India.

Dr Sabai Phyu
Head, BSL-3 laboratory/Biosafety Coordinator

Dr Sabai Phyu received her Bachelor of Medicine degree from the Institute of Medicine I, Yangon, Myanmar in 1986. In 1990, she received a Diploma in Tropical Medicine and Hygiene from Mahidol University in Thailand. She worked as a physician until she went to the University of Bergen, Norway for her PhD study in 1994. She received the Ph.D. degree in 2000. Her thesis was focusing on studying host immune responses against TB in mice. During her post-doc fellow time in the same university, she continued her research on epidemiology of TB and drug resistant TB, evaluating new TB diagnostic tools, and molecular typing of TB. She joined Novartis Institute for Tropical Diseases (NITD) in 2004 as a research investigator in the TB unit. She is currently Head of the BSL-3 laboratory/Biosafety Coordinator at NITD. In addition, she has been leading a team of Indonesian researchers who have been working on TB research at the NITD’s collaborative labs in Indonesia.

Ms Leong Kee Mei

Ms Leong Kee Mei received both her Bachelor of Science and Masters of Science degrees at the National University of Singapore. She has been involved in biosafety since 2004, when she joined the Biological Resource Centre (BRC) as a Biosafety Officer. She was tasked in setting the Health, Safety and Environmental Health management programme for the Animal BSL-3 Facility at BRC. She spent a year as the Senior Biosafety Officer in Lilly Singapore Centre for Drug Discovery before returning at the end of 2008 to become the Health, Safety and Environmental (HSE) Head in the Biological Resource Centre (BRC). She is currently a Committee member of the Biorisk Association of Singapore.

Mr David Lam is currently working as the Senior Manager, Biological Safety at the Singapore General Hospital. He is responsible for the implementation and maintenance of workplace safety & health management system in the areas of biological, laboratory and facility safety. Part of his work also involves in the design for safety of new and renovation of laboratory facilities in SGH. Prior joining SGH, David was working at the DSO National Laboratories as Biosafety & Facilities Manager. He was responsible for the testing and commissioning, operation & maintenance of the BSL2 & BSL3 laboratories. From 2005 – 2010, David served in the Asia Pacific Biosafety Association (A-PBA), as EXCO member. Currently he is the Vice-president of the Biorisk Association of Singapore (BAS).

Mr Wu Tuck Seng

Tuck Seng is the Deputy Director & Head of the Pharmacy Department, National University Hospital, Singapore. He holds a Master of Health Science (Management) degree from the University of Sydney (2005) and a Bachelor of Pharmacy (Honours) degree from the University of London, UK. He has worked as a hospital pharmacist for 24 years with professional interest in oncology, medication safety and clinical pharmacy.
He is the President of the first Singapore Pharmacy Council, and was Chief Examiner of the Singapore Pharmacy Council, Board of Examiners (2006 – 2008). He was the past president of the Pharmaceutical Society of Singapore from 1995 to 1997.
He holds and has held key appointments institutionally, nationally and internationally. Tuck Seng is the Chairperson of the National Medication Safety Steering Committee, Ministry of Health, Singapore, Chairperson of the NHG/NUHS/JGH Pharmacy Blueprint Workgroup, a member of the Ngee Ann Polytechnic Biotechnology Advisory Committee, and a member of Medical Professional Audit Committee, Dover Park Hospice. He is the secretary of the NUH Pharmacy & Therapeutics Committee, a member of the Expert Group of the National Healthcare Group (NHG) Medication Safety Collaborative, the NUH Ethics Committee and the NHG Domain Specific Review Board which oversees clinical trials. He is a member of editorial advisory board of the Journal of Pharmacy Practice and Research, Australia.
His many achievements include the following: he was the recipient of the 1994 Pharmaceutical Society of Singapore Delta West International Exchange Programme, the first Pharmaceutical Society of Singapore Professor Lucy Wan's Outstanding Pharmacist Award for his contributions to the profession in 1995, the Minister for Health Award for the Tsunami Relief Operation and The Courage Fund Healthcare Humanity Award 2005. He was conferred the commendation medal by the President of Singapore at the 2005 National Day Awards for work excellence and contribution to healthcare. Dr Hing Wee Chuan received MSc (Pharm) from National University of Singapore and is a Certified Processional in Healthcare Information and Management Systems. He is currently Principal Pharmacist (Informatics) of NUH and involved in NUH Medication Safety Office. Previously, he was a Drug Information Service Pharmacist at KK Women’s and Children’s Hospital for 9 years. After which, in 2005, he moved on to National University Hospital where he took on his current role as Informatics Pharmacist. In National University Hospital, he successfully implemented both the electronic inpatient medication record (eIMR) and inpatient pharmacy automation system (IPAS). He is currently in MOH National Medication Safety Task Force and is also involved in various committees such as, NUH Formulary Management Committee, High Alert Medication Task Force, Electronic Medication Record Project Management Office, Clinical Decision Support workgroup and Pharmacy Blueprint committee.

Dr Tai Hwei Yee's is a Senior Consultant in Anaesthesia at Tan Tock Seng Hospital (TTSH) and has a clinical background in Intensive Care Medicine. In 2000, she took on the administrative appointment as Director, Casemix Office, a post which she relinquished in 2010. She currently serves as Clinical Advisor to the Casemix Office.
While she was Director of Casemix, she implemented Singapore's first Case Management Unit, and was responsible for helping to drive various improvement activities to achieve better clinical outcomes for patients on complex care plans and clinical pathways. She was actively involved in the introduction of Chronic
Disease Management in National Healthcare Group (NHG), in training Case and Care Managers to assume pivotal roles in Chronic Disease Management programmes and supporting clinical champions to develop pathways for chronic care. In 2007, She was chaired the NHG Enterprise Data Warehouse project that put together the system to allow identification of patients with chronic diseases, create decision support for care management and outcomes research to improve care for this group of patients.
She established Singapore's first outpatient, nurse-based pre-operative evaluation service at TTSH to enable introduction of Same Day Admission (SDA) for elective surgery, and facilitate increased ambulatory surgical care. This has resulted in TTSH having one of the highest rates of DS and SDA amongst our public institutions.
In 2003, Dr Tai received the Medal of Valour from the government of Singapore for her role in supporting the national response in overcoming SARS. She coordinated the ICU services at TTSH to support management of patients affected by SARS and was the key clinician involved in design and equipping of the national post-SARS isolation facility.
She was appointed Assistant Chairman, Medical Board for Clinical Quality and Audit at TTSH in 2004. Dr Tai was instrumental in setting up the framework in TTSH for patient safety and has been the institutional sponsor and driver for various patient safety collaboratives – such as the Medication Error Collaborative, Communication of Critical Lab Results Collaborative and the High Alert Medication Collaborative, where she chairs the Expert Panel.
She has also helped to create a formalised hospital framework to address complaints and serious adverse events including pro-actively analyzing incidents, addressing patient's concerns, staff support system and Critical Incident Management. This work has resulted in a training programme for NHG, endorsed by the Medical Protection Society to instruct all clinicians on effective Open Communications skills. In 2010, Dr Tai was invited by the Hong Kong Hospital Authority to share TTSH and NHG's experiences in patient communications around adverse events and poor outcomes. In addition, she has been involved as facilitator and faculty member for NHG's Patient Safety programme, Clinical Practice Improvement Programme and Lean Transformation. She has been involved in CPIP teaching programmes in Thailand in 2010/2011 and Quality improvement mini-courses to an international audience at the International Forum in Quality and Safety in Healthcare since 2009.
In 2006, she was awarded the NHG Outstanding Citizenship Award for her administrative and clinical leadership in driving health process improvement, patient safety and cost effective use of medical technology in TTSH and across NHG. She is a member of the NHG Chronic Disease Management System Steering Committee, NHG Medication-Related Clinical Decision Support and Standards Steering Committee and the Electronic Clinical Documentation Steering Committee. She chairs the Adverse Drug Event Alert and Surveillance project for NHG – the first electronic clinical decision support system in Singapore for medication safety.
Dr Tai is also a member of the NHG Pharmacy and Therapeutics Committee, and the NHG Right Siting Committee. In 2008, Dr Tai took up concurrent appointment as Deputy Chief Quality Officer for National Healthcare Group where she continues to work to improve quality and patient safety across all the institutions within NHG.
At the National Level, Dr Tai contributes to quality improvement and patient safety as a member of the National Medication Safety Steering Committee where she is the Chairman of the Safe Medication Practices workgroup. This workgroup not only plans and advises on standardizing relevant safe medication practices, but also advises MOH on the alignment and integration of existing and future automated medication systems. Dr Tai is also a member of the MOHH Clinical Advisory Group for the National Electronic Health Record.

He was awarded the Ministry of Health's Healthcare Quality Improvement Fund (HQIF) to Reduce Medication Errors in KKH and to Reduce Healthcare-Associated Infection in The NICU.
He is also the hospital's Chairman of Medication Safety Committee and Director of Medical Informatics.

Ms Yasmin Ng

Yasmin Ng is a Principal Pharmacist working at Changi General Hospital (CGH). She graduated with a degree in Pharmacy (Honours) from the National University of Singapore and holds a Master degree in Social Science from the University of South Australia.
Presently, Yasmin oversees the operation at CGH outpatient pharmacy and is a member of the CGH Medication Safety Committee. She has been instrumental in the setting up of ambulatory clinics e.g. anticoagulation clinic, smoking cessation clinic, lipid clinic, and medication therapy management (MTM) service.
Under the Healthcare Quality Improvement and Innovation Funds, she led the team in the project Pharmacist's role in Medication Therapy Management in Outpatient Care Setting in 2008. The success of the project led to CGH being the first hospital to implement MTM service in Singapore.
Yasmin is also currently involved in the pioneering of Pharmacist Outreach Programme – a home based MTM service together with the ACTION (Aged Care Transition) team under Agency for Integrated Care.

Harbans Kaur obtained her State Registered Nurse Certificate in 1977 and had 10yrs experience in hospital setting in medical, infectious and dermatology nursing before pursuing her post basic in Community Health Nursing in 1988. She was awarded the HMDP Fellowship award in Community Geriatric Nursing and Promoting Continence Management in Australia for 3 months from March to June 1995.
She has vast experience in Community Nursing and Primary Health Care. Her deep passion lies in working and caring for the elderly, especially visiting the elderly in their own homes.
She joined AIC in November 2009 in the Care Integration Department of the Referral Management Team and ACTION. She provides support to the Action Team Care Coordinators at CGH and NUH.
She is also involved in this Pharmacist Outreach Program (POP) collaborating together with the hospital Pharmacists and CCs of the 5 RHs on medication management at home.

Ms Zahara Mahmood

Zahara Mahmood obtained her Bachelor of Arts (Social Work) at the National University of Singapore in 1991. She has twenty years of social work clinical experience in working with disadvantaged children, troubled youth, frail elderly persons and their caregivers. Her vast experience working in a special school setting to a community-based organisation, has spurred her to pursue her social work interest in a medical setting next. She joined Changi General Hospital as a Medical Social Worker in 2007 and is currently heading the Aged Care Transition (ACTION) team since October 2008.
Zahara is also an active volunteer with a registered charitable society, Casa Raudha Women Home, which provides shelter and care for abused women and their children since November 2009. She was also a volunteer facilitator to a Malay-speaking support group at the National Cancer Centre and currently she gives pro bono talks and training to voluntary welfare organisations.

Dr Kenny Tan

Dr Kenny Tan graduated from the NUS Yong Loo Lin School of Medicine in 1998. He subsequently obtained a Masters in Education (Policy, Leadership and Change) and has a Graduate Diploma in Occupational Medicine.
Dr Tan practiced at KK Women's and Children's Hospital and Singapore General Hospital. He also worked as a Clinical Research Fellow at the National Birth Defects Registry with the Ministry of Health.
With a keen interest in youth development and education, he then joined the Dean's Office, Yong Loo Lin School of Medicine, National University of Singapore (NUS YLLSoM) as Senior Associate Director (Admissions and Student Affairs). Since 2008, he has also been a tutor in Bioethics at the Centre for Biomedical Ethics, NUS YLLSoM.
Currently Director (Corporate Affairs and Special Projects) at the Ang Mo Kio – Thye Hua Kwan Hospital, Dr Tan believes in integrating Patient Care, Research and Education across institutions for the integration and betterment of patient care. In line with this perspective, he and his colleagues have partnered different institutions in patient care, research and education initiatives. This includes the Pre-Admission Patient Visits (PAPV) whereby the Hospital's PAPV team visit patients referred by Khoo Teck Puat Hospital (KTPH) and Tan Tock Seng Hospital (TTSH) prior to transfer to AMK-THKH. This initiative has enabled the AMK-THKH trans-disciplinary team to better prepare for patients' admission.
Dr Tan set up and is currently the Chair of the Hospital's Quality Management Office, looking into clinical and service excellence indicators.
Dr Tan is also an associate physician at Khoo Teck Puat Hospital.

Ms Lim Chien Fang

Chien Fang has a B.Sc. (Biology) and M.Sc. (Management). She joined NUH in Nov 2007. Her initial job scope was attending to outpatient and inpatient matters, including management of right-siting projects. She was subsequently fully assigned to right-siting and care integration matters across NUH.
Since 2008, Chien Fang has worked closely with St. Luke's Hospital and developed a number of collaborative projects. This was made possible with the signing of a MOU in July 2009. Since then, NUH and SLH have been working closely on matters related to patient care, funding, capacity building and infrastructure development. NUH and SLH was the first RH-CH pair to initiate the Subacute Funding Pilot administered by MOH.
